Abstract

Rochester Mayo Clinic is a complex primary care clinic with several valuable care management programs. A gap was noted in the communication between the inpatient and primary care practices when primary care patients were admitted to the hospital. A quality improvement project was implemented and a registered nurse clinical liaison (RNCL) role was developed. The RNCL is vital to ensuring patients transition smoothly from inpatient to outpatient and are connected to primary care.
