Commentary At the very heart of GI endoscopy lies the desire to see things more clearly than they have been seen before. The past 20 years have seen an explosion in our ability to see things in new and better ways through endoscopes, including EUS, narrow-band imaging, confocal laser endomicroscopy, and other techniques. VLE is a technique that uses optical coherence tomography with near-infrared light. The system typically uses through-the-scope catheters with balloons on them to self-center the catheter in the lumen. This case illustrates the use of VLE to characterize a recurrent duodenal NET. The lesion was imaged after it underwent EMR. The VLE image very closely mirrors the histopathologic findings seen on the specimen ex vivo. It would have been interesting to see whether this information would have changed the approach or performance of the EMR before the lesion was removed. This case illustrates the incredible resolution of VLE technology, which is still being used only to a limited extent in endoscopy. Douglas G.
