Abstract

Photodynamic therapy seems able to control high-grade dysplasia within Barrett's esophagus about 80% of the time. Long-term results are not available but the treatment is promising. Given the success with surgical intervention, however, use of photodynamic therapy should be reserved for nonsurgical candidates at the current time. The complications that occur with photodynamic therapy are not trivial and must be weighed against the potential benefits. Cutaneous photosensitivity, chest pain, nausea, and stricture formation all may occur. Improved dosimetry is needed to decrease these problems.
