Abstract

A steady functional decline in surviving nephrons and increasing nephron dropout underlie the progression of chronic kidney disease (CKD). A concept common to all mechanisms implicated in the progression of CKD is that processes centered in a particular renal compartment [en dash] glomerular, tubular, interstitial, vascular [en dash] recruit injurious processes in others, and it is this increasing recruitment and interplay of diverse pathobiologic processes, involving multiple renal compartments, that underlie progressive CKD. This chapter reviews these processes and their interaction in the progression of CKD, along with the role of specific mediators and inhibitors of CKD.
