Abstract
A 66-year-old man was admitted to the hospital with increasing shortness of breath of 14 days' duration and progressive swelling of his right leg during the preceding 7 hours. While the clinical diagnosis was deep venous thrombosis of the right leg with pulmonary embolism, two-dimensional echocardiography demonstrated a thrombus-in-transit (right-heart thrombus type A). After thrombolytic therapy with 100 mg of rTPA the patient's condition improved significantly and the thrombus disappeared completely. If in suspected pulmonary embolism echocardiography reveals a thrombus-in-transit thrombolytic therapy with rTPA can be applied effectively.
