Abstract

We thank Ms. Kucheryavaya and the United Network for Organ Sharing for their letter of date May 16, 2017 in response to our manuscript (Kransdorf EP, Pando MJ. Calculated panel reactive antibody with decimals: A refined metric of access to transplantation for highly sensitized candidates. Hum Immunol 2017; 78(3):252–256). In our manuscript we state that there was “an error in the CPRA generated by UNet based on the unacceptable HLA antigens listed for the candidate.” Ms. Kucheryavaya and the United Network for Organ Sharing indicate that they have investigated the four patients with discrepant unacceptable HLA antigens (UA-HLA) and calculated panel reactive antibody (CPRA) values and found that “The issue was caused by the authors comparing CPRA values computed using two different sets of unacceptable antigens: 1. CPRA value calculated by UNet based on unacceptable antigens at the time of organ offer and 2. CPRAd value calculated by authors based on unacceptable antigens at time of candidate's removal from the waiting list.” For our manuscript we utilized the listed unacceptable HLA antigens (UA-HLA) at the time of waiting list removal, and we calculated the CPRA with decimals (CPRAd) from these antigens. We agree that the four instances of disagreement between the UNOS CPRA and the CPRAd were due to changes in the listed UA-HLA between the time of calculation at transplantation of the UNOS CPRA and the time of waiting list removal (when CPRAd was calculated). Accounting for this, the UNOS CPRA calculations are entirely accurate. As such, we would like to retract the statement that there was an error in the CPRA generated by UNet as well as the data in Table S1 concerning the four patients with discrepant results. We apologize to the readers as well as the United Network for Organ Sharing for any inconvenience this may have caused.
